DC urges J form holder farmers to get enrolled under Sarbat Sehat Bima Yojna

Farmers can visit market committees/common service centres for SSBY Card

Jalandhar: Deputy Commissioner Vishesh Sarangal on Monday said that all the ‘J’ form holder farmers of the district must enroll themselves under the AB- Sarbat Sehat Bima Yojna to avail cashless treatment up to Rs 5 lakh in empanelled and government hospitals.

Disclosing this here today, the Deputy Commissioner said that all the farmers who have ‘J’ forms were eligible for deriving benefit under the scheme. He said that they need to submit their declarations along with the other requisite documents to the office of concerned Market Committee. He also added that the premium of the insurance of the farmers would be paid by the Punjab Mandi Board and the farming community would not have to pay any fee. Further, he said that the beneficiaries could avail treatment up to Rs. 5 lakh for various ailments including heart operation, cancer treatment, joint replacement and the operation in accident cases.

The DC said that the farmers desirous of availing benefits could get the self declaration form, from the concerned Market Committee office or the firm of the commission agent (Arhtiya). After receiving the applications from the farmers, the uploading would be done on the portal specially created for the purpose by Common Service Centres following which the eligible farmers will get Sehat Bima Cards.

District Mandi Officer Arvinder Singh also asked the commission agents to personally contact those farmers who have J-forms but not enrolled under the scheme.
